Community Support Program

The Community Support Program (CSP) is an assertive community based treatment program, staffed by a multidisciplinary team that assists adults with serious and persistent mental illness to live as independently as possible in the community.

Admission Criteria

·        Must be over the age of 18

·       Have Medicaid

·        Have a qualifying diagnosis of a chronic mental illness (i.e. Major Depressive Order, Bipolar Disorder, Schizophrenia, etc.)

·       Have a significant risk of either continuing in a pattern of institutionalization or living in a severely dysfunctional way if CSP services are not provided

·       Impairment in one or more of the following functional areas:

         à Vocational, education, or homemaking

         à Social, interpersonal, or community functioning

         à Self-care or independent living

·        Have been consistent and extensive efforts to treat the individual (such efforts have persisted for at least a year, except in unusual circumstances) causing the individuals' condition to move beyond the clinical standard of practice for outpatient services, traditional casement, and/or crisis. 

Exclusionary Criteria

·         Organic mental disorders (i.e. Dementia, Traumatic Brain Injury, etc.)

·         Mental Retardation

·         Primary diagnosis of drug abuse or dependence

Included Services

·         Medication management, monitoring and education

·         Psychiatry services

·         Community based case management services

·         Positive social opportunities

·         Resource coordination (i.e., housing, establishing a primary care doctor, dental care, obtaining benefits, food, clothing, transportation, etc.)

·         Continuing or completing educational/vocational goals

·         Assistance in learning and accomplishing skills needed to function more independently

·         Assistance with transportation

·         Assessment and treatment planning

·         Weeded and holiday coverage
