Community Health Improvement Plan

We heard from many Walworth County residents and organizations during our most recent Community Health Assessment (CHA). After hearing from the community, we prioritized the issues to create the 2022-2025 Walworth County Community Health Improvement Plan (CHIP). Below, you will find our CHIP website. This collection contains information about the CHIP process, all of the partners involved, data, and all of the goals we are working on. Vision: Improved health for all through connected communities where we live, learn, work, and play

Mission: Create a united vision for Walworth County that centers on building a culture of health.

Foundations of Health: These foundational elements will be addressed through the strategic priorities. As such, the goals, objectives, and outcomes in the plan will incorporate these foundations.
