Vital Records (Births, Deaths & Marriages)
Effective January 3, 2017, the State of Wisconsin has statewide issuance of vital records.
- Births - October 1, 1907 - Present.
- Marriage - October 1, 1907 - Present.
- Deaths - September 1, 2013 - Present. Death certificates prior to September 1, 2013 must be obtained at the occurrence or residence county only.
Applicants must have a direct and tangible interest in order to purchase a certified copy of any vital record.
Applications submitted in person in our office generally take 15 to 20 minutes to complete. We ask that you arrive by 4:45 p.m. to allow time to fill out the application. Note: at least one form of valid identification which contains your current name is required to complete each request.
Complete the appropriate form(s) below, include a copy of your form of identification, a self addressed stamped business sized envelope and applicable fee. In most instances, mail orders received are processed on the same day as received.
In most instances, online orders are processed on the day received or the day following. For online orders, please change the shipping method to U.S. (Standard) Mail if you do not wish to use the express mail service.
Please note: there are fees associated with payment by credit card which are added to the statutory copy fees.
