Lakeland health Care Center’s (LHCC) Social Services Department is responsible for independently identifying and documenting the psychosocial and emotional needs of residents.
Address complaints by residents or family members.
Addressing the psycho social needs of residents and their family members, and offer support.
Assist in discharge planning, including coordinating with community resources regarding alternate placement from LHCC to other community/health care facilities. Arrange resident/family care conferences.
Assist residents in maintaining their highest level of functioning and positive existence.
Collaborate with family and physicians to make hospice referrals when appropriate, and address end of life issues.
Partner with psychiatric providers to ensure that resident’s mental health needs are met.
Provide educational events to the residents, staff, and the community.
Provides information to residents/family regarding advance directives.
