All public health clinical services are offered by appointment only. These services are not limited to county residents. To schedule an appointment please contact us. Only cash, check, Medicaid (ForwardHealth/BadgerCare), Auxiant, and Medicare are accepted.

Report a Communicable Disease

Call our office immediately to report a Category I communicable disease at 262-741-3445. Category II and III diseases may be reported by phone, fax, or email. Find more information on how to report a communicable disease here.

Referral
