Participant Overview

The CCS program can provide a number of services needed which are paid through Medicaid. Services that fall under the Service Array and are part of the client’s approved Recovery Plan are fully covered. The CCS Service Array includes the following areas:         

  • Assessment                                                        
  • Recovery Planning                                           
  • Service Facilitation (Case management includes coordinating and monitoring services to ensure the individual receives timely and appropriate care)
  • Communication & Interpersonal Skills Training (Developing interpersonal skills, conflict resolution, and assertiveness)
  • Community Skills Development & Enhancement (Developing independent living skills)
  • Diagnostic Evaluations & Specialized Assessments
  • Employment Related Skills Training (Assistance in gaining and utilizing skills necessary to find and hold a job)
  • Medication Management
  • Physical Health & Monitoring (Managing side effects and symptoms of illness, including monitoring of vital signs)
  • Psychoeducation (Developing coping skills to manage the issues posed by mental illness and/or substance use disorder)
  • Psychotherapy
  • Recovery Education & Illness Management (Identifying supports for recovery and advocating for those supports)
  • Substance Abuse Treatment
  • Individually Tailored Psychosocial Rehabilitative Services

In CCS, the individual takes control of their treatment and recovery. Individuals enrolled in CCS work with a dedicated team to decide which programs and supports will assist the individual to reach their recovery goals.
