Connecting Communities

Every 3-5 years, we take a step back and ask our community what the most important health issues and needs are for Walworth County. We don't do this alone and involve many other groups, organizations, and agencies and look at a wide range of data.

We listen, learn, and identify priorities for partners across Walworth County to connect on in order to improve our community's health.

In August 2021, we are launching our next Community Health Assessment & Improvement Planning Cycle. There are many different ways you can get involved!

  1. Complete the Community Health Survey online using the link above or print the survey (English version, Spanish version) and mail it to:

    Attn: Division of Public Health
    1910 County Rd NN, Elkhorn WI
  2. Spread the word about completing the Community Health Survey.

Goals of the Community Health Assessment & Improvement Planning Process:

  1. Identify social & environmental policies and systems that impact health in our community
  2. Analyze health data to identify priorities for health in our community
  3. Assess how well entities in the community work together to create community wellness
  4. Create a strategic plan led by the community to improve health & wellness for all
