Community Health Assessment & Improvement

Connecting Communities

Every 3 years, we take a step back and ask our community what the most important health issues and needs are for Walworth County. We don't do this alone and involve many other groups, organizations, and agencies and look at a wide range of data.

We listen, learn, and identify priorities for partners across Walworth County to connect on in order to improve our community's health.

We heard from many Walworth County residents and organizations during our most recent Community Health Assessment (CHA). After hearing from the community, we prioritized  the issues to create the 2022-2025 Walworth County Community Health Improvement Plan. Using a community-based process, this plan aims to accomplish the following:

Vision: Improved health for all through connected communities where we live, learn, work, and play

Mission: Create a united vision for Walworth County that centers on building a culture of health.

Foundations of Health: These foundational elements will be addressed through the strategic priorities. As such, the goals, objectives, and outcomes in the plan will incorporate these foundations.

  • Built Environment
  • Economic Stability
2022-2025 Strategic Priorities:
  1. Build community & civic connection
  2. Improve access to care by centering on lived experience
  3. Unite policy & planning with health for all

What's Next?

Using the data collected from the Community Health Assessment, community leaders and organizations will partner together to create a strategic plan, led by the community to improve health and wellness for all. This plan is called the Community Health Improvement Plan.
