[ARCHIVED] Take the Community Health Survey Today: Deadline is Sept. 30

Connecting Communities health survey information

(ELKHORN, WI) – Walworth County Health & Human Services’ Division of Public Health is currently conducting a survey to hear more about what it is like to live, learn, work, worship, and play in Walworth County.

The survey is a part of Walworth County’s Community Health Improvement process. “Every three to five years, we take a step back and ask our community what the most important health issues and needs are for Walworth County,” says Walworth County Public Health Officer Erica Bergstrom. “Participating in the Community Health Survey allows your voice and the voice of your neighbors, friends, and family to be heard.”

This year’s process is focused on “Connecting Communities,” with an emphasis on our collective ability to positively impact the health and well-being of our community by intervening on a variety of levels.

Bergstrom states, “As we’ve seen over the last year and a half, our health is more interconnected than ever. Our health and well-being is so much broader than access to healthcare or health behaviors—it includes socioeconomic factors, mental health, transportation, housing, and so much more.”

Survey results will be used to identify needs, set goals, and work with community partners to improve the health and well-being of all who call Walworth County home. Focus groups will also be conducted with area organizations and community members to learn more about needs in Walworth County.

The Community Health survey is available in English and Spanish until September 30, 2021. It’s voluntary, anonymous, and takes less than 10 minutes to complete.   More information can be found at www.co.walworth.wi.us/CHA-CHIP.

Paper copies are available at Walworth County Health & Human Services (1910 County Rd NN, Elkhorn) or can be requested by calling (262) 741-3200.
